AFFIDAVIT OF DELAYED
REGISTRATION OF BIRTH

I, BALDOMERO P. COLASITO, Filipino, of legal age, single, and a

resident of No.313 Zone 5 Baras, Canaman, Camarines Sur, after having
been duly sworn in accordance with law, hereby depose and say:

1. That I am the applicant for the delayed registration of my birth;

2. That I was born in Sangay, Camarines Sur on November 7, 1958;

3. That I was delayed in registering my birth certificate for the reason

that my parents failed to register my birth;

4. That it was only recently when I applied for PSA authenticated birth
certificate that I found out that my birth was not registered;

5. That I am executing this affidavit to attest to the truth of the

foregoing facts and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 5th

day of March, 2020 at Naga City, Camarines Sur, Philippines.
