AFFIDAVIT

I, JOSE C. SICAT, of legal age, Filipino and a resident of BRGY.

Maimpis, City of San Fernando, Pampanga, after having been duly sworn
to in accordance with law, do hereby depose and state, that:

1. I am a duly elected incumbent Barangay Councilor of Brgy.

Maimpis, City of San Fernando, Pampanga;

2. I personally know BERNARDO TAPNIO y YANGA, who is a

resident of Brgy. Maimpis, City of San Fernando, Pampanga.
Being my constituent, I know him as a person who pursues a
better life;

3. As a person of good standing, I am willing to take into my

custody BERNARDO TAPNIO y YANGA pending the
provisional dismissal of his case should he be released on
recognizance by the Honorable Regional Trial Court Br. 42 of the
City of San Fernando, Pampanga;

4. I know my obligation, duty and responsibility as a custodian of

BERNARDO TAPNIO y YANGA. I likewise know that if he
fails to appear before the Honorable Court when so required, I
shall be held responsible and I may be cited in contempt of court;

5. Therefore, I hereby undertake to bring BERNARDO TAPNIO y

YANGA before the Honorable Court when his appearance is
required provided, I shall be notified of such an Order;

6. I am executing this Affidavit to attest to the truth of the foregoing

statements and for whatever legal purposes it may serve.
