I, JOSEPH TORRES LAMPANO, of legal age, Filipino citizen, and with residence
and postal address at Purok 2, Del Rosario, City of San Fernando, Pampanga, after having
been duly sworn in accordance with law, do hereby depose and say:

1. That I am the owner of J.T.L. Hydraulic Parts and General Merchandise, a business
enterprise duly registered with the Department of Trade and Industry (DTI) with
principal office at Purok 2, Del Rosario, City of San Fernando, Pampanga;

2. That I am occupying the above-mentioned office and conducting my business

activities therein without paying any rental or compensation because my
grandmother, Pacencia David, owns the said office;

3. That my grandmother has long acquiesced to my conduct of business activities in

her property without paying rental or any compensation;

4. That I am executing this affidavit to attest to the truth of the foregoing statements
and for whatever legal purpose it may serve.
