JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, DOLORES M. CEA and CHERRY I. JIMENEZ, both of legal age,

and residents of Zone 2, Brgy. Binanuaanan, Pili, Camarines Sur, under oath,
hereby depose and say, that:

1. We both personally know LUZ DE LIMA RAMOS, who were our

neighbor, friend and acquaintance, in our barangay in Zone 2, Brgy.
Binanuaanan, Pili, Camarines Sur;

2. That the true and correct middle name of LUZ RAMOS is “DE
LIMA” as appearing in her birth certificate issued by Local Civil
Registrar of Municipality of Pili as hereto attached;

3. That however, the middle name of LUZ DE LIMA RAMOS in the

payroll account of DSWD was erroneously indicated as
DESQUITADO;

4. That the LUZ DE LIMA RAMOS appearing on the birth certificate

and DSWD Payroll Account, although the latter is with erroneous
middle name, pertain to one and the same person;

5. That we are not related to LUZ DE LIMA RAMOS, within the

fourth civil degree of consanguinity and affinity;

6. We are executing this joint affidavit to attest to the truth of the

foregoing statements, for whatever purpose it may legally serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 13th day
of August 2019 at Pili, Camarines Sur.
