AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, JAIME BARTOLOME MILLENAS JR. filipino, married, of legal age

and ROSANNA F. DAYTO, filipino, single, of legal age, both residents of
Barangay Cabid-an, Sorsogon City under oath depose and state:

1. That we know personally MR. ELY D. FUNDALES, being a resident

of Barangay Cabid-an, Sorsogon City;

2. That MR. ELY D. FUNDALES and MR. ELIAS D. FUNDALES are one
and the same person who died single last October 5, 2019;

3. That we are executing this affidavit for the purpose of claiming the
Voter’s ID of MR. ELY/ ELIAS FUNDALES;

4. That we further attest that MS. ROSEMARIE D. FUNDALES is the

sister of MR. ELY D. FUNDALES, hence authorized to claim the
voter’s ID and process other documents of the latter;

5. That we are executing this affidavit to attest to the truth of the

foregoing statement and for whatever legal ‘purpose this may serve.
