AFFIDAVIT

I LUNINGNING J. ORTIZ, Filipino, of legal age, married and a resident of

Villamil St., Barangay Sirangan, Sorsogon City, after having been duly sworn to in
accordance with law hereby depose and state:

1) That I am the mother of Nissa J. Ortiz who was born on January 12, 1989;

2) That my daughter was Baptized and received the Sacrament of

Confirmation at Sorsogon Cathedral, Diocese of Sorsogon;

3) That upon inquiry at the office of the Parish of Sorsogon Cathedral, it

appears that there is no record of such baptism and confirmation of my
daughter;

4) That I am executing this affidavit to include the name of my daughter in the

official list of individuals who were baptized and received the Sacrament of
Confirmation in the Diocese of Sorsogon;

5) That I am executing this affidavit to attest to the truth of the foregoing

statement and for whatever legal purpose this may serve.

We, JOSEPHINE D. LANUZA, Filipino, of legal age, married with

residence and postal address at Villamil St., Barangay Sirangan, Sorsogon City and
FERNANDO L. DISCAYA, Filipino, of legal age, widow and a resident of
Villamil St. Barangay Sirangan, Sorsogon City after having been duly sworn to in
accordance with law hereby depose and state:

1) That we are the godparents of Nissa J. Ortiz, who was born on January
12, 1989 at Barangay Sirangan Sorsogon City;

2) That Nissa J. Ortiz was baptized in the Cathedral of Sorsogon (Diocese

of Sorsogon) on the year 1989;

3) That as godparents we are present and witnessed said ceremony of

baptism of Nissa J. Ortiz;

4) That we are executing this affidavit to attest to the truth of the foregoing
statement and for whatever legal purpose this may serve.
