AFFIDAVIT OF GUARDIANSHIP

I NIDA H. DOLINDO filipino, married with residence and postal address at

Barangay Sampaloc, East District, Sorsogon City, under oath depose and say:
1) That my niece Roxanne H. Delima, 23 years of age is planning to enter
into a contract of marriage with Reynaldo P. Dolindo

2) That the Philippine law on marriage provides that a party to a contract who
is 23 years of age must obtain the advice of both parents prior the
celebration of marriage;

3) That the mother and father of Roxanne H. Delima was estranged from
each other and we have no information as to their whereabouts for almost
10 years now;

4) That due to the absence of her parents I am acting as her guardian and I
am giving my consent for her to enter into a contract of marriage;

5) That I am executing this affidavit to attest to the truth of the foregoing

statement and for whatever legal purpose this may serve;
