AFFIDAVIT OF NO PENDING CRIMINAL INVESTIGATION or

PROSECUTION

I, GRACE D. DIVINA, Filipino, of legal age, single and resident of Buhatan, East
District, Sorsogon City Philippines, after having been sworn to in accordance with the law
hereby depose and say:

1. That I was a former employee of the City Government of Sorsogon with a

position as Executive Assistant II at the Office of the City Administrator;
2. That effective November 1, 2018, I resigned from the said office;
3. That there is no pending criminal investigation or criminal prosecution filed
against me;
4. That I hereby authorize my former employer the City Government of Sorsogon to
deduct all financial obligations that is pending thereof prior to my resignation if
there be any; and
5. That this authority is my own free will and I attest that no one forced, coerced,
threatened or intimidated me to execute this instrument; and
6. That I likewise attest to the veracity of the foregoing facts executing this Affidavit
for whatever legal intents or purposes it may serve.
