AFFIDAVIT

I, ROSANA L. GALANG filipino, married ,of legal age and a resident

of Washington Drive,Legazpi City under oath depose and say:

1. That I was planning to take the Licensure Examination for Teachers this
year in furtherance of my professional career and also have to renew my
license as professional nurse which expired last 2001;

2. That the entries of data in my Certificate of Live Birth which is an essential

requirement must be corrected since there has been a discrepancy in the
spelling of my first name and date of birth;

3. That I was using the name ROSANA BARBACENA LABRADOR (maiden

name) since birth and my date of birth is June 1, 1972 but what appeared
in my birth certificate is ROSANNA BARBACENA LABRADOR and the
recorded date of birth is August 23, 1972;

4. That in order to correct the discepancies in my birth certificate I was

required by the Local Civil Registrar of the City of Sorsogon to obtain a
medical records from the hospital where I gave birth to my children;

5. That since I was using natural medicine/traditional food supplement there

was no instance were I was hospitalized except when I gave birth;

6. That as of this moment said hospital no longer exist in Legazpi City and
the medical records was destroyed due to typhoon Reming;

7. That Iam executing this affidavit in lieu of the medical records which I was
not able to produce;

8. That I am attesting to the truth of the foregoing statement and for whatever
legal purpose this may serve;

I, HELEN A. JANAPIN filipino, married ,of legal age and a resident of

Purok 1, Barangay Piot, Sorsogon City under oath depose and say:

1. That as of the present time, I am not employed in any capacity at any

office, corporation, company nor self-employed;

2. That I am executing this Affidavit of Non-Employment to attest to the truth

of the foregoing and to support my application to file for Correction of
Name in the Certificate of Birth;
