AFFIDAVIT OF EXPLANATION

I, JERB MOLINA NIALLA, of legal age, single, Filipino, and a resident of

BLK 20 Lot 6, 11th Avenue, HHSG NorthSignal Village, Taguig, after having been duly
sworn to in accordance with law, do hereby depose and say;

1. That I plan on being issued a passport;

2. As such, I asked my aunt to assist me in producing the necessary

documents for the issuance of the same;

3. That when all of my documents are complete, I received an Email from

the Department of Foreign Affairs that I have successfully
confirmed my passport appointment booking;

4. Said email contained instruction for me to follow, which I validly

complied with;

5. Subsequently I received an Email from the Department of Foreign Affairs,

informing me that my application for the issuance of a passport is
suspended because my prior passport was undeclared;

6. It was then that I was informed by my aunt of her failure to declare said
passport because she was unaware of the existence of the same;

7. That I am executing this affidavit to attest to the truth of all the foregoing,
and as basis for the issuance of a new passport, and for all legal
purpose it may serve me.
