AFFIDAVIT

I, SURAIDA FERMIL ESTRADA, of legal age, Filipino, and a resident of 24

Padilla St. East, Poblacion, Lingayen, Pangasinan, after having been duly sworn to in
accordance with law, do hereby depose and say that;

1. I admit the allegations stated in Jennifer C. Gumapos’s affidavit, but deny ever having
ordered tupperwares from her and failed to pay the same;

2. Likewise, I deny her allegation that I owe her Php 42,900.00, because on March 17,
2022, we have entered into an amicable settlement wherein we both agreed that I am to
pay Php 17,500.00 on a monthly basis starting on March 31, 2022. A copy of the
Amicable Settlement made in the Office of the Lupon Tagapamayapa dated on March 17,
2022 is herein attached and marked as Annex “A”

3. After the execution of the settlement in the Barangay, I made installments on the
succeeding months as agreed in our settlement, among which was made in the Office of
the Punong Barangay of Barangay Poblacion, Lingayen Pangasinan dated on April 1,
2022, a copy of which is herein attached and marked as Annex “B”;

4. Likewise, I paid the succeeding installment payments to her in her office as evidenced
by a written acknowledgement receipt by Myla Bugarin, who is an authorized
representative of Jenifer Gumapos herein attached and marked as Annex “C and series”;

5. That I am executing this affidavit to attest to the truth of all the foregoing, and for all
legal purpose it may serve.
