after having been duly sworn to in accordance with law, hereby depose
and say:

1. That I am an applicant for a loan under the Agricultural

Competitiveness Enhancement Fund (ACEF);

2. That such application shall be submitted with the Landbank of

the Philippines;

3. That I have no outstanding loan from any formal lending

institution for the same project being applied for; and

4. That I am executing this affidavit to attest to the truthfulness of

the foregoing and to support my application for the loan
abovementioned.
