I, JUAN DELA CRUZ, of legal age, Filipino, and with residence and postal
address at Dagupan, Pangsinan, do hereby appoint JUAN DELA CRUZ 2, of
legal age, with residence and postal address at Dagupan, Pangsinan as my true
and legal representative to act for and in my name and stead and to perform the
following acts:

1. To process on my behalf the reformation of the DEED OF ABSOLUTE

SALE executed between JUAN DELA CRUZ as buyer and Philippine National
Bank as seller so that it will reflect the true agreement of the parties which is to
include in the sale the improvements in the parcel of land covered by TCT No.
020-2010000237; and

2. To transfer Tax Declarations Nos. 2014-00-0019-00919 and 2014-00-

0019-00918 under my name, JUAN DELA CRUZ from that of Imelda (former
owner).

Hereby granting unto my representative full power and authority to

execute and perform every act necessary to render effective the Deed of
Absolute sale in my favor, JUAN DELA CRUZ, as though myself has performed
it and hereby approving all that he may do by virtue hereof with full right of
substitution of his person.
