Human Resources, Administrative, Legal and Purchasing (HALP) Division and
authorized person of CITISTORES, INC., do hereby name, constitute and authorize,
nameof representative, of legal age, Filipino to be my true and lawful attorney-in-
fact, to act on my behalf and in my name and of CITISTORES, INC, nameof
representative being the postition of Centro Department Store -branch, is
hereby vested with the following powers and authority:

1. To appear, act, sign, and submit documents for and on my behalf and of
CITISTORES, INC. and/or its business unit Centro Department Store
before the PROSECUTOR’s OFFICE of Sta. Cruz, Laguna, as our official
representative in the matter of filing and pursuing the THEFT case against the

Shoplifting incident happened on date and time. Stolen item/s amounted to

HEREBY GIVING AND GRANTING unto my said attorney-in-fact full powers

and authority to do and perform all and every act requisite or necessary to carry into
effect the foregoing authority, as fully to all intents and purposes as I might or could
lawfully do if personally present.
