I, Rachele Tanjutco-Pili, a Filipino citizen, married, of legal age, with residence and postal address at 219
Almendrala St., Barangay San Vicente, City of San Pedro, Laguna, after having been duly sworn in
accordance with law, hereby declare that:

1. I am a Filipino Citizen from birth until the present;

2. I am in Possession of a Philippine Passport issued by the Department of Foreign
Affairs;
3. That I have not obtained any foreign citizenship nor took an oath of allegiance to any
foreign country.
4. I am executing this affidavit to inform the proper authorities of the name above stated
facts in the lieu of my application for Australian Awards of Citizenship, to attest to the
veracity of truth of the foregoing and for all legal intents and purposes this may serve
best.
