duly sworn to according to law hereby depose and say:

1. That, I am the same RHODAN REX M. ARIZALA whose name appears on the
certificates of employment, job trainings, and association affiliation issued by
previous employer and association/society in which I am bonafide member;

2. That, said certificate of employment, trainings and association affiliation

certificates were legally obtained and secured by the herein affiant;

3. That, these certificates/pertinent documents are in need to be authenticated by

lawful authorities as valid requirements for traveling abroad;
