AFFIDAVIT
(Attesting to the Fact of Employment)

I, MYCA O. CONDES, of legal age, Filipino, with residence and postal address at
Block 11 Lot 32, Camella Homes 1, San Pedro, Laguna, after having been duly sworn to
in accordance with law, do hereby depose and say that:

1. That I am a Registered Nurse by profession;

2. That I am in the process for travel and work abroad and one of the
requirements is to present my previous employment and/or trainings
relative to the job I am processing to;

3. That on October 14, 2019 I was given a Certificate of Employment as

Nurse 1 of the Patient Care Services Division from March 16, 2017-
October 11, 2019, signed by the OIC-Human Resource Developmnt
Department- Ms. Editha H. Ricarte and Hospital Director- Atty. Edwin L.
Domatatac, M.D.FPCP,MHA,CESE. Machine copy of the Certificate of
Employment is hereto attached as part of and marked as “ANNEX A”;

4. That on April 26, 2017 I was issued a Certificate of Employment as

Private Duty Nurse from February 2012 up until September 11, 2013,
signed by Violeta Alambra. Machine copy of the Certificate of
Employment is hereto attached as part of and marked as “ANNEX B”;

5. That on March 17, 2012 I was given a Certification by CLINIC SYSTEMS,

INC. as Nurse-Trainee from September 26, 2011 up until February 10,
2012, signed by Francisco E. Motillano, Administrator. Machine copy of
the Certification is hereto attached as part of and marked as “ANNEX C”;

6. That on March 17, 2012 I was given a Certification by CLINIC SYSTEMS,

INC. for undergoing a Training Program for HealthCare Provider from
September 26, 2011 up until October 18, 2011, signed by Francisco E.
Motillano, Administrator. Machine copy of the Certification is hereto
attached as part of and marked as “ANNEX D”;

7. That on September 2011 I was given a Certification by Xavier Medical

Center as Staff Nurse from June 7, 2011 up until September 26, 2011,
signed by Dra. Donna Jean B. Aniñon, Medical Director. Machine copy of
the Certification is hereto attached as part of and marked as “ANNEX E”;

8. That on June 13, 2011 I was given a Certification by the Human Resource
Management Office of the Local Government of San Pedro for being a
Volunteer Nurse assigned at the Gavino Lying-In Clinic from 27th day of
July 2010 until 26th day of October 2010 and 1st of February 2011 up
until 30th day of April, 2011, signed by Leah Lu-Morando, OIC, HRMO.

marked as “ANNEX F”;

9. That on February 22, 2011 I was given a Certificate by Health Trends

Medical Clinics @ Diagnostic Center assigned as CLINIC/MOBILE/ON
CALL NURSE from November 8, 2010 up until February 22, 2011, signed
by Walter A. Dimayuga, M.D., Medical Director and Katherine A.
Occidental. RN, Chief Nurse. Machine copy of the Certification is hereto
attached as part of and marked as “ANNEX G”;

10. That on 27th day of July, 2015 I was given a Certificate of Employment by
Franciscan Missionaries of Mary for being a Nurse assigned at the
Infirmary from February 15, 2010 up until July 15, 2010, signed by Sr.
Zoriada Robiso, FMM. Machine copy of the Certificate of Employment is
hereto attached as part of and marked as “ANNEX H”;

11. That I execute this instrument in order to attest the truth of the above
facts and for all legal intents and purposes this may serve.
