AFFIDAVIT OF EMPLOYMENT

I, JOSHUA N. LOZANO, of legal of age, Filipino citizen, married, and a resident of

No. 1144 Bliss, Bonuan Binloc, Dagupan City, Pangasinan, after having been duly sworn to
in accordance with law hereby depose and say:

1. That I am a Registered Nurse by Profession;

2. That I was employed at Specialist Group Hospital and Trauma Center, Tapuac
Dist., Dagupan City, Pangasinan, as a Nursing Assistant in our from 100- bed capacity
tertiary level Hospital at the Operating room and Delivery room department from April 1
2008 to March 31, 2009 then as a Staff Nurse from April 1, 2009 to May 7, 2011. A copy
of said Certification of Employment issued by Thelma G. Arenas, RN. MAN., and Carlito V.
Arenas, M.D. Medical Director, is hereto attached as reference;

3. That I was also employed National Medical Services Co. Ltd. Dammam Medical
Dispensary, Saudi Arabia from June 30, 2011 up September 8, 2012. A copy of said
Certification of Employment issued by Abdullah Safar Alghamdi, Managing Director, is
hereto attached as reference;

4. That I was also employed at Defense Complex Hospital, Bab Al Yemen, as a

Staff Nurse in Operating Room Department from July 5, 2013 up to December 31, 2013.
A copy of said Certificate of Employment issued by Samir Al- Sanafi, HR Manager and Dr.
Hesham Abdo Othman, General Director, is hereto attached as reference;

5. That I was also employed at DR. Sulaiman Al Habib Medical Services Group
Holding, Riyadh Saudi Arabia, as a Registered Nurse – 2 in Cath Lab Department from
May 10, 2014 up to February 22, 2018. A copy of said Service Certificate and Clearance
issued by Abdulrahman Bin Mohammed Bin Eyed, General Director, is hereto attached as
reference;

6. That I am executing this Affidavit to attest the truthfulness of the foregoing

statements and for whatever legal purpose it may serve.
