AFFIDAVIT

I, APRIL JOY GAMONES URSABIA, of legal age, single, Filipino and a Zone 7 Bayabas, ,
Cagayan de Oro City. After having been sworn to in accordance with law, hereby depose and
say, THAT;

1. I am a Registered Nurse with professional Regulation Commission I.D no.

0855095 and presently employed as a nurse at the J.R Borja General Hospital,
Carmen, Cagayan de Oro City.

2. I started working as a Job Order at the said hospital on February 12, 2016 and
assigned in the actual work as nurse in pediatrics department and in the
emergency rooms.

3. I am about to tender my resignation as a nurse with the above mentioned

hospital which would take effect sometime in July 1, 2018.

4. Since my employment as a nurse, I earned an excellent performance and has no

derogatory record.

5. I execute this affidavit to attest to the truthfulness and veracity of the foregoing
and for whatever legal purposes it may serve.
