2. That being an employee of Baguio General Hospital and Medical
Center, Baguio City I was issued a Certification as Staff Nurse from
April 1, 2009 to August 15, 2012;
3. That I hereby attest to the authenticity and genuineness of the
document/s attached made an integral part;
4. That I hereby agree to be liable and answerable to perjury and/or
falsification of public documents in the event that the document/s
is/are found not in order;
5. That I am executing this affidavit to attest to the truth of all the
foregoing facts and circumstances and for all legal intents and
purposes it may serve.
