1. That I am a licensed NURSE in the Philippines with PROFESSIONAL IDENTIFICATION

CARD NO. 0879220 issued by the Professional Regulation Commission (PRC);

2. That I was employed in Dr. Sulaiman Al Habib Hospital Arrayan at P.O. Box 100266 Riyadh
11635, Khurais Road, Kingdom of Saudi Arabia as a Staff – Nurse 3 from October 1, 2017 until
September 30, 2019. I have attached hereto my Service Certificate and Clearance issued by Dr.
Sulaiman Al Habib Medical Services Group Holding.

3. That the document attached in this affidavit is a certified true copy and faithful reproduction of the
original;

4. That I guarantee the authenticity and genuineness of the said document;

5. That I shall be responsible and liable for the submitted document if the same found spurious or
tampered upon verification, validation and authentication by any private or government offices/s;

6. That I am executing this affidavit to attest to the truth of the foregoing statements and for the
purpose of complying with the requirements for my work application abroad.
