address at Penaranda St., Legazpi City as a STAFF NURSE from September 29, 2014 up to
present;

3. That the documents attached in this affidavit are certified true copy and faithful reproduction of the
original;

4. That I guarantee the authenticity and genuineness of the said document;

5. That I shall be responsible and liable for the submitted document if the same found spurious or
tampered upon verification, validation and authentication by any private or government offices/s;

6. That I am executing this affidavit to attest to the truth of the foregoing statements and for the
purpose of complying with the requirements for my work application abroad.
