AFFIDAVIT

I, NELSON B. SEVERIÑO, JR., Filipino, of legal age, with office address

at 7/F Santolan Town Plaza Offices, 276 Santolan Road,
San Juan, Metro Manila 1500, after having been duly sworn to in accordance with
law, hereby depose, state, and warrant that:

1. I am the Government Liaison Officer of PANELO LAW OFFICE (the

“Firm”), a single proprietorship duly organized under Philippine law with principal
office address at 7/F Santolan Town Plaza Offices, 276 Santolan Road, San Juan,
Metro Manila 1500;

2. This Affidavit forms and is an integral part of the documentary

requirements for the accreditation of law offices and paralegals with the Bureau
of Immigration (the “BI”) (collectively, the “Application”);

3. I attest that I do not have any relative working with the Bureau of
Immigration (BI) within the third civil degree of consanguinity or affinity.

4. I undertake to allow the BI, or any of its authorized representatives,

to verify any and all documents which are necessary for the review and approval
of the Application;
