AFFIDAVIT OF DENIAL

after having been duly sworn to in accordance with law, hereby depose
and state:

1. That when I filed an application for clearance at the National

Bureau of Investigation I was advised that the said office could
not issue the required clearance in view of the appearance of
name similar to my name in NBI Files involving the following
cases:

2. That the said person who is the accused in the above-mentioned

cases is not the same person as the affiant who carries the name

3. That I have never been charged of any offense and or violation

of existing laws here in the Philippines;

4. That I am executing this affidavit to attest to the foregoing facts

and to deny categorically and specifically under oath that I am
not the same person mentioned in the docket/files of NBI with
the said derogatory criminal records;

5. In view of the foregoing, I am requesting NBI Office to issue

the appropriate NBI clearance in my favor for compliance with
the certain requirements.
