I, MARY JOY S. ENDO, of legal age, Filipino with residence and postal address at B8
L20 Rupee Street Villa Carolina 1, Brgy. Tunasan, Muntinlupa City, Metro Manila, after
having been duly sworn to in accordance with law, do hereby depose and say:

1. That I am an applicant for enrollment for school year 2020-2021 in University of

the Philippines, Diliman, Quezon City;

2. That I am not enrolled to any school, college, or University herein local and in
abroad;

3. That one of the requirements of the said University is for me to submit an

affidavit attesting to the fact of I am not enrolled to any School, College or
University here in the Philippines nor in abroad.

4. That if there is concealment of previous enrollment, the said University of the

Philippines enrollment becomes null and void;

5. I execute this instrument in order to attest the truth of the above facts, and for all
the legal intents and purposes this may serve
