AFFIDAVIT OF ACCOUNTABILITY

I, ____________________________, of legal age, single/married/widow/widower, Filipino and a

resident of _____________________________________________, after being sworn in accordance with
law, hereby depose and state that:

1. I am presently employed as ____________________________ at Bacolod Columbia Marketing, Inc.

(BCMI), a domestic corporation duly organized and existing under and by virtue of the laws of the
Republic of the Philippines, with principal office address at #2525 Narra Extension, Hervias
Subdivision, Brgy. Villamonte, Bacolod City, Negros Occidental;

2. I understand that the property/properties listed and attached in a separate sheet of paper (Annex A),
has been loaned to me by BCMI. I am therefore expected to exercise due care in my use of this
property/properties and to utilize such property/properties only for authorized purposes. Negligence
in the care and use will be considered cause for disciplinary action, which may result in discharge
from employment;

3. Likewise, I will be responsible for any damage done (excluding normal wear and tear), and I will
replace any items issued to me that are damaged or lost at my expense;

4. I also understand that BCMI’s property/properties must be returned at the time of my separation from
employment or when it is requested by my manager or supervisor; and that I will be charged for any
property/properties issued and not returned to BCMI;

5. I hereby authorize BCMI, a payroll deduction to cover the replacement cost of any item/s issued to
me that is not returned for whatever reason, or is not returned in good working order; and

6. I am executing this Affidavit as a proof of good faith in complying with the rules on accountability
issued by BCMI and for whatever legal purpose/s it may serve.
