AFFIDAVIT OF CLAIMS
(Re: ACCIDENT ON M/V VIRGINIA I, January 17, 2017)

I, JOWELL F. FERRANCO, of legal age, single, Filipino citizen

and with residential address at Block 6, Lot 12, Phase 3, Dubai
Street, Vista Verde North Exec. Village, Caloocan City, after having
been sworn to in accordance with law hereby depose and state;

1. That I am the authorized representative of HOPE METAL

( HAUWE), who was the contractor for the delivery of:

1,100 Sets of 3PC SECONDARY

CHAIR/TABLET/SUPPORT-GREE

2 That said products were bound for delivery to various

schools under the Department of Education (Dep-Ed) at ABORLAN,
NARRA, PALAWAN, who was the end-user thereof;

3. That on January 07, 2017, as representative of said

contractor, I caused the delivery of said cargoes, consisting of 55
bundles and loaded at M/V VIRGINIA I, anchored at Delpan pier,
Tondo, Manila, owned and operated by VIRGINIA SHIPPING LINES
CORPORATION with address at 0436 Capt. Cooper Street, San
Jose, Occidental Mindoro as evidenced by the Delivery Receipt
hereto attached as ANNEX “A” and Bill of Lading No. 10198,
showing payment of P26,400, which is hereto attached as ANNEX
“B”;

4. That on January 16, 2017 at around 9:00 P.M, said M/V

VIRGINIA I with the above stated cargoes loaded, departed for
Roxas, Palawan;

5. At around 1:00 AM of January 17, 2017 said M/V

VIRGINIA I caught fire which resulted in its sinking Southwest of
Caballo Island, approximately eight (8) nautical miles from Fortune
Island, Nasugbu, Batangas, a copy of the Coast Guard report
consisting of two (2) pages is hereto attached as ANNEX “C”;

6. That the sinking of said M/V VIRGINIA I on January 17,

2017, carried with it, the above stated cargoes costing P500,000.00
inclusive of delivery and transportation costs, and other cargoes from

other shippers, with no traces of its contents, thus, the declaration of
its total loss beyond recovery;

7. That I am executing this Affidavit in order to attest to the

truth of the foregoing facts and circumstances and in order to serve
notice of our claim from whatever insurance proceeds, if any, and for
whatever legal purpose this may serve.
