AFFIDAVIT OF GUARDIANSHIP

I, FLORA CASTILLO NORIESTA of legal age, married, Filipino citizen and a

resident of and with postal address at Barangay San Francisco B, Lopez, Quezon,
Philippines, after having been sworn in accordance with law, hereby depose and say:

That, I am the maternal aunt/guardian of JELIAN RESSURECCION ESTRADA,

born on September 06, 2006 in Lopez, Quezon;

That, on February 18, 2023, JELIAN RESSURECCION ESTRADA gave birth to

JACE LIAM ESTRADA at Magsaysay Hospital at Lopez, Quezon;

That, on the year 2017 the biological father of JELIAN RESSURECCION, left
their conjugal dwelling, and from then up to present, we have no knowledge of his
whereabouts;

That, the biological mother of JELIAN RESSURECCION ESTRADA, Mary Jane

Alano Ressurreccion, my cousin, is currently working as Domestic Helper at Kingdom of
Saudi Arabia;

That, JELIAN RESSURECCION ESTRADA, is under my care, support and

custody since January 2022 up to now;

That, I executed this affidavit to prove that I am the guardian of JELIAN

RESSURECCION ESTRADA, being his maternal aunt.

That, I executed this affidavit to prove the veracity of the foregoing facts and in
support of my application for the issuance of PhilHealth Number and the registration of
the Certificate of Live Birth of JACE LIAM ESTRADA at the Local Civil Registry of
Lopez, Quezon.

IN WITNESS WHEREOF, I have hereunto set my hand this 20th day of February
2023 at Lopez, Quezon.
