AFFIDAVIT
I, CHARINA JUACALLA SANTOS, of legal age, Filipino, married, with residential
and postal address at Sitio Mahabang Tanaw, Tinamnan, Lucban, Quezon, Philippines, after
having been sworn to in accordance with law hereby depose and state that:

1. I am the mother of JOHN RODNEY JUACALLA SANTOS born in Lucban, Province

of Quezon, Philippines on October 3, 1996;

2. The father of JOHN RODNEY JUACALLA SANTOS is RICARDO D. SANTOS,

JR., a resident of San Pablo City, Laguna, who is absent and of unknown whereabouts
since 2015 or more than seven (7) years now;

3. I am the only parent of the child who is capable of giving my consent to the said marriage
and to sign all the necessary documents in relation thereto.

4. My child is intending to contract marriage with MICAH ELLA LAGUMBAYAN in the

City of Tayabas, Province of Quezon, Philippines;

5. I am giving my consent to my child JOHN RODNEY JUACALLA SANTOS, marrying

MICAH ELLA LAGUMBAYAN;

6. I am executing this Affidavit for the purpose of informing the authorities concerned of
the veracity of the foregoing facts and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hands this 25th day of May 2022 in
the City of Tayabas, Province of Quezon, Philippines.
