AFFIDAVIT OF ACKNOWLEDGEMENT OF PATERNITY

I, RENANTE N. LABAGUIS, of legal age, Filipino. And presently residing at

Tayabas City, Quezon having been duly sworn to in accordance with law, do hereby
depose and say:

1. That I am the biological father of the child ENRICO MANUEL S. MIRAS;

2. That the biological mother of the child is RUTH MIRAS - DUMALASA;
3. That our child was born on May 12, 2002 at Bambang, Nueva Vizcaya.
4. That at the time of the birth of our child, I was not legally married to her biological
mother;
5. That I hereby acknowledge my paternity of the child, ENRICO MANUEL S.
