AFFIDAVIT

That I, NORMA S. CAMRAL, of legal age, Filipino, married and a

resident of Barangay Lawili, Aleosan, Cotabato, after having been duly
sworn in accordance with law, do hereby depose and say:

That I am the guardian/grandmother of JAY BUAYABAN

CAMRAL who was born on September 24, 2021 at Barangay Lawili,
Aleosan, Cotabato to parents Narciso S. Camral and Rosalinda C.
Buayaban;

That JAY BUAYABAN CAMRAL was abandoned by her mother,

Rosalinda C. Buayaban since he was three (3) months old;

That I intend to register the birth of JAY BUAYABAN CAMRAL

but the mother could not give her consent because we have no more
personal knowledge of her whereabouts;

That I execute this affidavit in order to attest to the truth of the

foregoing statement and for whatever legal intents and purposes the
same may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 1 st
