AFFIDAVIT OF CONSENT

We, LOYD CATAMIN and RUBY ANN CATAMIN of legal age, Filipino, and
a resident of Poblacion, Bagumbayan, Sultan Kudarat, after having been duly sworn in
accordance with law, do hereby depose and say:

1. That we are the father and mother respectively of

____________________who was born on _________________ at Bagumbayan, Sultan
Kudarat;

2. That Mr. Ariel C. Miranda and Myrna C. Miranda have signified their intention
to legally adopt our aforementioned child and will file the necessary Petition for
Adoption in the proper Court;

3. That we are giving our full consent for the adoption of my child, in fact, we
have already surrendered custody of our child to the above-mentioned spouses;

4. That we have difficulty of meeting the needs of said child due to limited
earning plus we have two other children to support that we could not further support to
rear the said child;

5. That we believe that it is to the best interest of our child that we allow the
adoption of our to the above-mentioned spouses because they could properly provide her
with her needs and could give her the love, care and attention which we could not
provide due to our present condition;

6. That it is to be understood that we gave our consent, without force, threat or

intimidation, to the adoption of our child without any reward or monetary consideration
but the consent of adoption of our child done for her best interest and for a better future;

7. That we will not, in any manner whatsoever, disturb the custody of our child
with the aforementioned spouses and we will not ask that we take back the custody and
parental authority over our child but instead we wish that the custody and parental
authority be transferred to the aforementioned spouses;

That we executing this affidavit to attest to the truth of the foregoing facts.
