AFFIDAVIT OF CONSENT AND AFFIRMATION OF SOLE CUSTODY RIGHTS

I, TTTTTTTTTTTTTTTTTTTTTT, Filipino, of legal age, and a resident of Cebu

City Philippines after having duly sworn to in accordance with the law, hereby
depose and state:

1. That I am the biological father of MMMMMMMMMMMMMMMMMMMMM, who

was born on November 25,2000 at Tagbilaran City, Bohol;

2. That I was married on ___________________ in a church ceremony to

GGGGGGGGGGGGGGGGGGGGGGGGGGGGG however, in our union, due to
our individual differences, we separated in fact and I am currently working
for five years already at Cebu City, my home city;

3. That my daughter is in actual custody to my ex-wife and her family, and I do

not object to the said set-up as I believe it would be for her best interests;

4. That I hereby give my consent and confirm that the sole custodian of my
daughter, is her mother, GGGGGGGGGGGGGGGGGGGGGGGG, and that I
allow her to live together at UK, for her best welfare and interests;

5. That I may not be the best and perfect father, but, all fathers would always
has good intentions for his daughter, and that all these things are for her
good future;

6. That I vouched to the truthfulness of my statements, and to further confirm

the sole custody rights in favor of GGGGGGGGGGGGGGGGGGGGGGGG.
