AFFIDAVIT

That I, CATHERINE S. NEO, of legal age, Filipino, married and a

resident of Villarica, Midsayap, Cotabato after having been duly sworn in
accordance with law, do hereby depose and say:

That I am the mother of KEVIN LEX S. NEO, who will be an incoming

first year college student of University of the Philippines Visayas, at Iloilo City;

That my son Kevin Lex S. Neo passed and qualified the scholarship
program offered by the University of the Philippines;

That I have existing personal loans with the following banks and
institutions which am I am continously paying monthly amortization payments as
follows:
