AFFIDAVIT OF NON FILING OF INCOME TAX

I, ELIENITO DIONESIO SARMIENTO,, of legal age, Filipino, married but separated,

with postal and residence address at Blk 56 Lot 9, Manganese St. Golden City Subd, Anabu II,
Imus City, Cavite, Philippines, after having be duly sworn to in accordance with law hereby
depose and say:

That I am the father of GREHILLE SHININIIH GRILLA SARMIENTO who is an

incoming first year college;

That I am a missionary. As such, I do not receive any income. I only receive monthly
allowances from the church on a voluntary basis. The amount of Php12,500,00
Philippines Currency monthly’

That as a result, I am exempted from paying income tax and is required to file an income
tax return;

That I am executing this affidavit of Non Filing of Income Tax to support the application
of my daughter for scholarship at the DE SALLE UNIVERSITY DASMARINAS CITY,
CAVITE, attest to the truth and veracity of the foregoing statement and for whatever legal
purposes;
