AFFIDAVIT

I, JOLLEOUS E. SACEDA, of legal age, single, residing at

Purok 8, Canaway, Iligan City, after being sworn in accordance with
law, depose and say that:

1. I am a job worker of the City Government of Iligan earning the

minimum wage of Three Hundred Thirty Eight Pesos per day;
2. I am the bread-winner of the family having to support my live-in
partner and two young children;
3. I have learned that I was one of the persons listed as prospect
SAP beneficiary to be validated by CSWD;
4. On May 4, 2020 I have seen my name listed at the bulletin
board of barangay Tibanga, Iligan City indicating that I am one
of the qualified recipient of the SAP Aid and was given my SAP
Card (See Annex A);
5. On May 7, 2020 I have received the amount of Six Thousand
Pesos (P 6,000.00) from CSWD;
6. Prior being given by SAP aid, I was asked by the CSWD
representative if I am a member of UCT and 4Ps of which I
responded no;
7. That I am executing this affidavit to attest to the truth of the
foregoing statement and to support for the dismissal of the case
filed against Barangay Captain Anduyan and ___________.
