AFFIDAVIT OF LESS INCOME

I, RUAIDA A. JAJIMIN, of legal age, Filipino and a resident of Barangay

Aguada, Isabela City, Zamboanga Peninsula, Philippines, after having been sworn to in
accordance with law, do hereby depose and declare:

1. That I personally know the person of LEANN SALMA J. MAMA, she being
my daughter and an applicant for Educational Service Contracting (ESC), which is a
scholarship at Claret College of Isabela;

2. That I am a government employee and a solo parent with a monthly salary of

only twelve thousand pesos (Php 12,000.00). My eldest child is already in third year
college hence my income is only sufficient for our daily needs;

3. That although I file my Income Tax Return with the Bureau of Internal
Revenue Office, I do not pay income tax since my income falls within the threshold of a
minimum income earner;

4. That I am executing this affidavit to attest to the truth of the foregoing

narration of facts, which is necessary to support the application of my son for
Scholarship and for whatever legal purpose this may serve best.
