AFFIDAVIT OF WAIVER TO APPEAL DECISION

I, APLE MAE ABALOS NAFUERZA, of legal age, married, Filipino and a

resident of Barangay Alangan, Limay, Bataan under oath depose and say that:

1. I am one of the employees of the Municipality of Limay, Province of

Bataan who appealed the disapproval of our appointments by Civil Service
Commission (CSC) Field Office Bataan;

2. On December 6, 2019, I received a copy of Decision from CSC Regional

Office III denying our appeal;

3. I am no longer interested in pursuing my appeal to CSC Main Office;

4. I am relieving and discharging the Local Government of Limay, Bataan

and its administrators from obligations, claim and other rights of action, monetary
or otherwise, pertaining to any claim or action which I may have by virtue of my
disinterest in pursuing my appeal;

5. I am voluntarily executing this Affidavit, free from force, coercion and

intimidation, to attest to the truth of the foregoing and for whatever legal purpose
this may serve; and

FURTHER, I sayeth none.

Bataan who appealed the disapproval of our appointments by Civil Service
Commission (CSC) Field Office Bataan;
