FIRST CODICIL TO THE LAST WILL AND TESTAMENT OF

RAIZA D. SUNGGAY

I, Raiza D. Sunggay, a resident of Santiago Norte, San Fernando City, La Union,

make this First Codicil to my Last Will and Testament dated 05 September 2022.
I. I hereby revoke Paragraph VII of the said Will, and in its place and stead
substitute the following Article:
“VII. I hereby designate Atty. Jenny B. Dimaculangan as the
executor and the administrator of this Last Will and Testament, and
in her incapacity, I name and designate Mr. Rigor D. Malakas as her
substitute.”

II. In Paragraph II, “my mansion at Forbes Subdivision, Makati, Metro Manila,
Philippines” shall be deleted therein, hence, it will no longer be given to
my husband, Mr. Mark Anthony G. Fortes. Instead, I authorize my
administrator, Atty. Jenny B. Dimaculangan, to sell it any day after my
death and give all the proceeds thereof to Saint Mary Orphanage,
Catbangen, San Fernando City, La Union.
IN WITNESS WHEREOF, I have signed this Notarial Codicil in the presence of
attesting witnesses on the 17th day of September 2022. Upon being first duly
sworn, I declare that this is the First Codicil to my Last Will and Testament dated
05 September 2022, containing of two (2) pages, signed by me in the presence of
three credible witnesses, and signed by three (3) credible witnesses in my
presence and in the presence of each other. I sign this codicil willingly as my free
and voluntary act for the purposes of disposing of my property at my death, and
that I am Twenty-Six (26) years of age, of sound mind and memory and that I am
under no constraint or undue influence.

ATTESTATION CLAUSE
We, the undersigned attesting witnesses, do hereby certify and attest that
the above and foregoing instrument composing of two pages was signed, sealed,
published and declared by Raiza D. Sunggay as and her First Codicil to her Last Will
and Testament dated 05 September 2022 in our presence and we, at her request
and in her presence and in the presence of each other, have hereunto subscribed
our names as attesting witnesses this 17th day of September 2022.

Beforeme, the undersigned authority, on this day personally

appeared Raiza D. Sunggay, Lucia D. Peralta, Cory C. Quirino and Daniel P.
Razon known to me to be the Testator and the witnesses, respectively, whose
names are subscribed to the annexed or foregoing instrument in their respective
capacities, and all of said persons duly sworn and declared to me and to the said
witnesses in my presence that said instrument is her First Codicil to her Last Will
and Testament, and that she had willingly made and executed it as her free and
voluntary act and deed for the purposes therein expressed, and the said witnesses,
each on his/her oath stated to me, in the presence and hearing of the said
Testator that she had declared to them that said instrument is her First Codicil to
his/her Last Will and Testament, and that she executed same as such and wanted
each of them to sign it as a witness; and upon their oaths each witness stated
further that they did sign the same as witnesses in the presence of each other and
in the presence of the Testator and at her request, and that said Testator at that
time possessed the rights of majority, was of sound mind and under no restraint.
Subscribed, acknowledged and sworn to before me by Raiza D Sunggay,
Testator, and Lucia D. Peralta, Cory C. Quirino and Daniel P. Razon, witnesses,
this 17th day of September 2022.
