b. Death Certificate of the deceased Mr. Herminio G.
Dumaguin.

2. In connection with the above references, the undersigned would like to
request Voluntary Financial Assistance for the above-named father of FO3 Lloyd F
Dumaguin, personnel of this station, who died last 20 February 2023 due to
Aspiration Pneumonia.

3. Further, the collected monetary assistance in whatever amount shall be

sent to LBP Account SA 1397 0214 39.

a. Memorandum signed by CSUPT RIZZA D SIMBAJON,
DSC, Regional Director, BFP Region 1, dated 24 October
2022 re: above subject; and
b. Invitation Letter of the Association of Regional
Government Information Officer (ARGIO) dated 19
October 2022 re: above subject.

2. In connection with the above references, be informed that the Association

of Regional Government Information Officer (ARGIO) will conduct a two-day annual
capacity training for government information officers dubbed as “Training Workshop
on Risk Communication, Newswriting in Ilokano and Mobile Journalism” on 08-09
November 2022 at Ariana Hotel in Bauang, La Union.

3. A registration fee of Three Thousand Five Hundred Pesos (Php3,500.00)

payable in cash or in check will be charged for each participant to defray expenses
for accommodation, food, honorarium of speakers, training kits, and other
miscellaneous expenses that will be incurred during the training.

4. Relative to this, you are hereby enjoined to inform your respective station
for any interested personnel to participate in this significant event. For further
questions and/or inquiries, you may coordinate with the Secretariat through Ms.
Caren Grace Cabanayan of PIA Region 1 (607-9240 or 0919-095-7149) or through
Assistant Regional Director Freddie G. Lazaro (0918-807-4763).
