PURACAN, CEO VI, Chief, BFP with subject DISTRIBUTION
AND TURNOVER CEREMONY OF 86 UNITS AMBULANCE
(TYPE 1-BASIC LIFE SUPPORT) dated December 2022;
and
b. Approved List of Distribution of 86 units Ambulance.

2. ICOW the above references, you are hereby directed to register all ambu-
lance in your area of responsibility (AOR) for licensing and accreditation
with the Department of Health - Health Facilities and Services Regulatory
Bureau NLT ______ thru this email _________.

3. Submit to the Office of Chief, Health Services Section the following docu-
ments, to wit:

4. Further, attached herewith are the forms to be filled out and submitted to-
gether with the other documentary requirements.
