SUBJECT: IMPLEMENTATION OF THE USE OF APPLICATION FOR LEAVE

FORMS

1. This is in connection with the implementation of the Quality Management

System based on ISO 9001:2015 wherein the publication and use of all leave forms
application and requirements are required.

2. In view thereof, you are hereby directed to observe the full implementation of
the newly designed format of all application for leave forms (Travel Abroad, Rehabilitation
Leave, et. al.).

3. Further, the new format of leave applications, monthly leave report and
requirement forms for leave transactions are now downloadable at the BFP official
website located at the downloads section.

4. For information, widest dissemination and strict compliance.
