BFP Region 10 re: Submission of Duty Detail of Deployment of Personnel to Identified
Quarantine Control Points.

2. Please be informed that Ozamiz City Fire Station is presently conducting


Oplan Ligtas Pamayanan (OLP) on the Road to twenty-one (21) barangays within our
AOR, seven (7) of which are visited twice per month. The said barangays are targeted
for OLP on the Road because such are high-risk barangays and located within or
proximity from the city proper. In addition, majority of these barangays have wide area
of geographic territory.

3. Further, it has been noted that petroleum fuel prices has been constantly
increasing

4. Risograph copies of various forms for fire code fees collection as well as for
fire safety inspections including After Inspection Reports and Inspection Order.

1. Reference: Memorandum dated 17 November 2021 from Regional Director,


BFP Region 10 re: Request for Submission of Detailed Summary of Remittance from
FY 2014-2020.

2. ICOW the above reference, please be informed that this station has lost its
ONCOL Payment Slips (ONCOL) and List of Collections (LC) from FY 2014 to 2015.
This is due to the improper turnover of records by the previous collecting agent to the
recent collecting agent. Thorough search and inquiries has been done to find the
aforesaid records to no avail.

3. To comply with the above-mentioned memorandum, this station has sent the
attached letter to Landbank Ozamiz City Branch dated 22 November 2021 requesting a
copy of ONCOL and LC from FY 2014 to 2015. However, in its attached letter reply
dated 24 November 2021, Landbank Ozamiz was not able to provide the said request
because they have already disposed the said records as mandated by National
Archives of the Philippines (NAP). They may be able to provide a bank statement for
remittance reconciliation but this would take months since the account is not maintained
in Landbank Ozamiz branch.

4. In this regards, the undersigned regretfully informs that this station can only
provide the ONCOL Payment Slips and List of Collections for FY 2016 to 2020. Please
be assured that the undersigned has already given strict instructions and taken up
appropriate measures to avoid loss of important records of fire code fees collections
and remittances.

Sabong of FO2 Abundio E Allanic II.

2. In connection with the above references, please be informed that the


undersigned immediately directed SFO1 Jayshree R Mendez, fire/arson investigator of
this station, to conduct investigation pertaining to FO2 Allanic’s addiction to Online
Sabong. Based on the attached investigation report, the investigator concluded that
FO2 Allanic is not engaging or playing to any Online Sabong nor addicted to such due
to the absence of any evidence to prove such matter. Further, based on the testimonies
of Annex Fire Sub-Station personnel, FO2 Allanic has been continuously rendering duty
and effectively performing his designation as Driver/Pump Operator.

3. In addition, the undersigned has been diligently monitoring all his personnel
regarding the status and performance of their functions and designations. Any issues
concerning personal matters of the BFP personnel is not the undersigned’s concern as
long as it does not affect his or her performance as a public servant. Please be assured
that you will be immediately and personally informed by the undersigned if any issues
will arise in this station that are beyond the undersigned’s capacity and capability in
order to effectively resolve such issues.

2. Upon the assumption of the undersigned as Acting City Fire Marshal, it has
been noted that the following fire trucks of this station have not been registered from the
Land Transportation Office:

3. To verify the official registration status of the above-mentioned fire trucks, the
undersigned requested system records of such fire trucks to the LTO Ozamiz through
the attached letter dated 19 November 2021. Attached system records from LTO show
that the said fire trucks have no motor vehicle records, an indication that they are not
registered with the LTO. In addition, the Nissan Tanker’s plate number (SDK-972) is
assigned to other motor vehicle.

4. On January ___, 2022, the undersigned met with Salome M. Navarro,


SVTRO-Chief, LTO Ozamiz City discussing the status and procedures for the
registration of the fire trucks. According to the SVTRO-Chief, only the accredited
importers/dealers and motor vehicle owners are authorized to initially register a motor
vehicle with the following general requirements:

a. Original Sales Invoice


b. Original LTO copy or electronically transmitted appropriate insurance
Certificate of Cover (Third Party Liability)
c. Original copy of Philippine National Police - Highway Patrol Group
(PNPHPG) Motor Vehicle (MV) Clearance Certificate and Special Bank

5. Upon inquiry and verification from this station’s current and previous supply
officers, the concerned fire trucks were delivered to this station with no attached
documents whatsoever necessary for its initial registration neither the registration itself.

6. In this connection, please be informed that the subject fire trucks are still not
registered up to this date. Said fire trucks are also still being utilized in responding fire
alarms. Much as the undersigned wanted to initially register said fire trucks but he is
constraint to do so because of the absence of any documents, as well as funds, in
support thereof.

twelve (12) hours upon receipt regarding the complaint against him on unethical
conduct that was lodged to the Presidential Complaint Center’s 8888 Citizens’
Complaint Hotline dated June 17, 2022 with Ticket Reference Number W20220617-
748-29.

3. Attached herewith is the memorandum reply from FO2 Buaron dated 21 June
2022.

4. In addition, the undersigned personally called FO2 Buaron and the


complainant, Sweet Mary T. Dampor on 21 June 2022 both expressing that the
particular complaint was already settled between them (attached are the screenshots of
call logs from the undersigned’s personal cellphone number). Also, the complainant
texted the undersigned to his personal cellphone number stating further that her
complaint has already been settled (attached is the screenshot of the said text
message).

disregarding lawful orders from the higher office especially directives from the Chief,
Bureau of Fire Protection.

3. Para VI, sub para D of the above-mentioned Memorandum Circular states


that “…It shall be the duty of the Administrative Officer to furnish the Regional Director
of a monthly report of the demerits earned by the unit.” From this directive of the
monthly report of delinquency, the undersigned interpreted that only on instances that
this station issues demerits that a monthly report will be submitted. Since from the
issuance of the said Memorandum Circular up to present, this station has not received
any demerit report filed by any of its BFP personnel. Hence, the absence of any monthly
report of delinquency.

4. The undersigned acknowledged his lapses of misinterpreting the directives


from the Chief, BFP. Please be assured starting this month that a monthly report of
delinquency will be submitted by this station to the appropriate office on time.

1. Reference: Memorandum dated 17 November 2021 from Regional Director,


BFP Region 10 re: Request for Submission of Detailed Summary of
Remittance from FY 2014-2020.

2. ICOW the above reference, please be informed that this station has lost its
ONCOL Payment Slips (ONCOL) and List of Collections (LC) for FY 2014. This is due
to the improper turnover of records by the previous collecting agent to the recent
collecting agent. Thorough search and inquiries has been done to find the aforesaid
records to no avail.

3. To comply with the above-mentioned memorandum, this station has sent the
attached letter to Landbank Ozamiz City Branch dated 22 November 2021 requesting a
copy of ONCOL and LC for FY 2014. However, in its attached letter reply dated 24
November 2021, Landbank Ozamiz was not able to provide the said request because
they have already disposed the said records as mandated by National Archives of the
Philippines (NAP). They may be able to provide a bank statement for remittance
reconciliation but this would take months since the account is not maintained in
Landbank Ozamiz branch.

4. In this regards, the undersigned regretfully informs that this station cannot
provide the ONCOL Payment Slips and List of Collections for the month of September
to December FY 2014. Please be assured that the undersigned has already given strict
instructions and taken up appropriate measures to avoid loss of important records of fire
code fees collections and remittances.

2. In connection with the above references, please be informed that FO2 Rhick
Anthony Doone B Buaron, Ozamiz City Fire Station, was on vacation leave on 01-29
October 2022 as reflected in his application for leave duly endorsed to and approved by
OPFM, BFP Misamis Occidental.

3. After the exhaustion of the above-mentioned leave, subject personnel failed


to report to this station which prompted the undersigned to issue the attached first
Return to Work Order dated 03 November 2022 directing subject personnel to return to
work within seventy two (72) hours upon receipt. Such order was served to the
residential address of FO2 Buaron at Purok 2, Barangay Maular, Aloran, Misamis
Occidental and was received thereat by certain Michael B. Celestian on the same date.
4. After the lapse of the first Return to Work Order, subject personnel still failed
to report to this station. Subsequently, on 07 November 2022, the undersigned issued
the attached second Return to Work Order which again directed FO2 Buaron to report
to this station within seventy two (72) hours upon receipt of the said order. Such order
was also served to the above-mentioned residential address of FO2 Buaron and was
received thereat by certain Michael B. Celestian on 10 November 2022. After the
seventy two (72)-hour deadline of the second Return to Work Order, subject personnel
still failed to report to this station up to this date.
