AFFIDAVIT OF ATTESTATION

I, ZENAIDA AGLIBOT JUBILADO, of legal age, Filipino and a

resident of Ilang-Ilang St., Sacrifice Valley, Hermosa, Bataan, under oath,
hereby depose that and say:

That I am the biological mother of KRYZZIAH MEIH AGLIBOT, 14 years

old, a minor who executed an AFFIDAVIT TO USE THE SURNAME OF
THE FATHER (AUSF) over her child NIÑO AGLIBOT LUMAGBAS and I
hereby declare that I personally assisted her in execution of the said
AUSF and that I hereby attest that she is fully aware of the legal
consequence of its execution;

That I am executing this affidavit in order to inform the authorities

concerned on the truth of the foregoing facts and for whatever legal purpose
it may serve.
