AFFIDAVIT OF CORRECTION
(RE: One and The Same Person)

I, INTONG M. LIGARAN, of legal age, Filipino citizen and a resident of Sumisip, Basilan
Province, Philippines, after having been sworn to an oath in accordance with the law do hereby
depose and say that:

1. I personally know SAMMY IMBAN LIGARAN who was born on JUNE 07, 1978 at
UPPER BENEMBENGAN SUMISIP, BASILAN PROVINCE, PHILIPPINES, to spouses
TIYONG ARABIN IMBAN and LIGARAN TELENG MUSTAJAB, considering that he is
my brother;

2. However, due to inadvertence and/or oversight, our mother’s name was erroneously
entered/recorded as TIYONG ASJAD MIMBAN on my Certificate of Live Birth when in
truth and in fact her true and correct name is TIYONG ARABIN IMBAN;

3. Our father’s name was erroneously entered/recorded as LIGARAN MAARAB

MUSTAJAB on my Certificate of Live Birth when in truth and in fact his true and correct
name is LIGARAN TELENG MUSTAJAB;

4. I hereby certify that, TIYONG ASJAD MIMBAN and TIYONG ARABIN IMBAN refer
to one and the same person;

5. I hereby certify that, LIGARAN MAARAB MUSTAJAB and LIGARAN TELENG

MUSTAJAB refer to one and the same person;

6. I make these statements under oath and in good faith in order to establish the fact that our
mother’s true and correct name is TIYONG ARABIN IMBAN and our father’s true and
correct name is LIGARAN TELENG MUSTAJAB ;

7. That I execute this affidavit in order to attest to the truthfulness of foregoing narration of
facts and for whatever legal purpose this may serve best.
