WITHDRAWAL OF APPLICATION FOR CORRECTION OF VOTER’S ENTRY

I, ALIM TAIBA MUNJAJAL, of legal age, Filipino citizen and a resident of Ungkaya
Pukan, Basilan Province, Philippines, after having been sworn to an oath in accordance with
the law do hereby depose and say that:

1. I have been known and have been using the name TAIBA ALIM MUNJAJAL as
appearing in all of my records;

2. However, there was a discrepancy in my name on my first record with the Office
of COMELEC, Ungkaya Pukan, Basilan Province, particularly, my name was
erroneously entered/recorded as SAIBA, when in truth and in fact my true and
correct name is TAIBA;

3. I hereby certify that TAIBA ALIM MUNJAJAL and SAIBA ALIM MUNJAJAL
refer to one and the same person;

4. I hereby withdraw my application with the COMELEC, Ungkaya Pukan, with

my registered name “SAIBA ALIM MUNJAJAL”

5. I make these statements under oath and in good faith in order to establish the
fact that my true and correct name is TAIBA ALIM MUNJAJAL;

6. That I execute this affidavit in order to attest to the truthfulness of foregoing

narration of facts and for whatever legal purposes this may serve better.
