AFFIDAVIT OF NON-FILING OF INCOME TAX RETURN

We, Spouses DENNIS C. EFONDO and FELIZA L. EFONDO, both of legal age,

Filipino citizens and residents of B95 L8 Hyacinth St., P6 South Fairway Homes, Landayan, San
Pedro, Laguna after having been duly sworn to in accordance with law hereby depose and state
THAT:

1. We are the legitimate parents of DAVID L. EFONDO, a senior high school

graduate last year and currently a college student of PUP Sta. Mesa;

2. That, my aforesaid son is applying for a college scholarship at Iskolar ng Lungsod

ng San Pedro and one of the requirements for such application is the presentation
of Income Tax Return (ITR) of his parent, which we have none, hence, this
affidavit;

3. That we are not gainfully employed with any private or public entity, thus, we are
not filing any Income Tax Return with the Bureau of Internal Revenue;

4. That the husband is earning money as a Jeepney Driver while his spouse is a
housewife.

5. That I am executing this affidavit for the purpose of attesting to the foregoing fact
and for Scholarship Application and for other legal purposes which this affidavit
may serve.
