AFFIDAVIT OF CONSENT

We, A and B, parents of G and h, Filipino citizens, of legal ages, and residents
of ________________________, after having been duly sworn to in accordance with law,
do hereby declare that:

1. We are applying for Japan Visa which are necessary and required for our family
vacation this year;

2. That we are willing to maintain and support our children financially, as shown
by the bank certificates, and other supporting documents that are attached to
our Korean Visa application; and, further guarantee that they will not become a
public charge or burden during their stay in the foreign country which will be
travelling.

3. We are executing this affidavit to attest to the truth of the foregoing facts and
for any legal purpose this affidavit may serve.
