hereby depose and state that:

1. I am still single and have never been married to anybody, neither do I have a
common-law-spouse;

2. I have not had any child;

3. My name does not appear in the Civil Registrar General on National Registry of
Marriages;

4. I am executing this Affidavit for the purpose of establishing the fact that I am still
single and no child and never been married;

5. That I am executing the foregoing Affidavit of Being Single in order to attest to the
foregoing facts and statements and to support my application for enlistment in the Armed
Forces of the Philippines.
