AFFIDAVIT

I, AILEEN B. ALBAY of legal age, married, Filipino, and a resident of

Dualing, Aleosan, Cotabato, after having been duly sworn in accordance with
law, hereby depose and say:

That I am the husband of SPO4, Jessie C. Albay assigned at the Aleosan

Police Station, Philippine National Police, Aleosan, Cotabato;

That sometime on the month of May 2009, my husband SPO4 Jessie C.

Albay gave me his application for sick leave prepared by SPO4 Goldilinie L.
Cantoy from May 5, 2009 to June 12, 2009 because he was hospitalized due to
kidney problem;

That I have submitted his application for sick leave at the Philippine
National Police, Provincial Office, at Amas, Kidapawan City;

That I could not specifically recall the name of the PNP personnel who
received the application of the sick leave of my husband SPO4 Jessie C. Albay
but I recalled that he received the copy at the provincial head quarters;

That due to length of time I could not recall where I placed and kept the
received copy of the application for sick leave of SPO4 Jessie C. Albay and I of
the honest belief that it was lost among the files;

That I exerted diligent effort to locate the received copy of the application
of sick leave but the could not be found or located;

That I execute this affidavit in order to attest to the truth of the foregoing
statements and for such other legal intents and purposes the same may serve.
